Team UOP Working Together form

Throughout your time at the University of Portsmouth, there is always a helping hand ready for you. The Department of Sport and Recreation, deliver sport, health and fitness to you, our students. Sport and Recreation look after 34 sports clubs, collectively known as Team UOP. Across everything we do, we deeply care about your wellbeing, particularly within Team UOP. 

We understand that at times, unique situations can arise and we want you to feel confident that we're here to stand by you and support your welfare every step of the way. If you’re able to, we want to provide sources to give you the tools to help yourself too.

This form is used for students working together with Sport and Recreation staff when you are concerned about yourself or have concerns about a student, committee or sports club who are in Team UoP.

  1. Fill out the Student Wellbeing Service 'Raise a Concern' about a student formThis goes straight to the University’s Duty Practitioner so they can make contact with the person who is seeking support
  2. Fill out this form (the Sport and Recreation’s Team UoP ‘Working Together’ form) so our welfare staff in Sport and Recreation can get in touch with you and offer support and guidance from a Team UoP sports club perspective
Extra Info:
  • This Working Together form will go straight to welfare staff in Sport and Recreation
  • Anyone can complete the 'Raise a Concern form' and 'Working Together form'
  • If you’re concerned about someone, let them know you’ll ask the Student Wellbeing Service to give them a call and that you’re working with Sport and Recreation to help
  • Check out Sport and Recreation’s web pages on Team UoP’s welfare and safeguarding for more advice once you’ve submitted both forms

  • Negative or inappropriate behaviour or misconduct by Team UoP members or clubs
  • A particular incident you were part of or witnessed
  • Club culture
  • Initiations or forced activities

Then come back to complete this Sport and Recreation 'Working Together' form so our staff in Sport and Recreation can contact you.

Unless you feel it would add to any risk, it is best if the student is made aware that we have been contacted. If you are concerned about this or aren't sure about how best to do this, this can be discussed with Zoe Monk (Sports Development Officer - Student Experience) via Zoe.Monk@port.ac.uk.

A complaint including issues, inappropriate behaviour, initiations, club culture or behaviour


Please be as specific as possible and try to avoid using nicknames

If so, please state their name and committee position

If the incidents happened over a period of time, please include details about when it started, how long it's been happening for and the frequency of any incidents

If the incidents happened across multiple places and venues please provide all addressess
